Myocardial deformation as a stratification tool in HCM

Background/Introduction Sudden cardiac death (SCD) is a major issue in hypertrophic cardiomyopathy (HCM) management, and its risk prediction stays challenging for clinicians. Ventricular arrythmias (VAs) have to be treated rapidly, indeed VAs lower prognosis HCM patients. Current ESC guidelines cardioverter-defibrillator implantation include left atrial (LA) whereas ventricle (LV) ejection function (LVEF) incorporated AHA latest guidelines. We explore the potential role LA LV strain as more sensible parameters rhythmic prognostication. Purpose To assess stratification patients accurate markers SCD. Methods Patients with newly diagnosed were enrolled three French University Hospital (Marseille, Bordeaux Rennes) between 2007 2018. At inclusion we realized comprehensive clinical exam, biological analysis, an echocardiography including two four apical-chambers, MRI. Composite primary endpoint (PE) analysis was occurrence related adverse events: ventricular tachycardia (VT), ICD therapy arrhythmia, or all-cause death. Occurrence dates endpoints retrieved by patient’s medical records phone calls all family members. Results mean follow-up 57 (34–82) months, events (n = 52) presented impaired deformation function. After multivariate found significant association our composite P 0,023 HR 0,95. revealed correlated PE 0,012 2,48. Conclusions Myocardial appears relevant marker which could linked sudden HCM. These might new calculators.
